(16 February 1999), pp. 338-342.&lt;/i&gt;&lt;br /&gt;&lt;br /&gt;Motivated by a search for improved quality and efficiency, increasing numbers of hospitals and physicians are moving from systems in which all primary care providers manage their own hospitalized patients or rotate this responsibility among themselves at infrequent intervals to voluntary or mandatory systems in which patients are &#34;handed off&#34; to the care of an inpatient physician, the &#34;hospitalist.&#34; All hospitalists manage medical patients in the hospital. Other potential roles for these physicians include triage in the emergency department, transfer of &#34;out-of-network&#34; patients, management of patients in the intensive care unit, preoperative and postoperative management of surgical patients, and leadership in hospital quality improvement and regulatory work. Hospitalists may add value by being more available to inpatients, having more hospital experience and expertise, and having an increased commitment to hospital quality improvement compared with primary care providers. Potential disadvantages of the hospitalist model include loss of information as a result of discontinuity of care, patient dissatisfaction, loss of acute care skills by primary care physicians, and burnout among hospitalists. A variety of models of care are needed to meet the clinical, organizational, financial, and political demands of diverse health care systems. The favored model should be that which produces the best clinical outcomes and the highest patient satisfaction at the lowest cost.</description>

(March 2006), pp. 88-93.&lt;/i&gt;&lt;br /&gt;&lt;br /&gt;BACKGROUND: Despite the dramatic growth of hospitalists, no studies have evaluated the type and frequency of activities that hospitalists perform. To evaluate the types and frequency of activities that hospitalists perform during routine work, we conducted a time-motion study of hospitalist physicians at our institution. DESIGN: A research assistant shadowed hospitalist physicians for 3- to 5-hour periods. Observation periods were distributed in order to sample all parts of a typical day of a hospitalist, including both admitting and nonadmitting periods. Activities were recorded on a standardized data collection form in 1-minute intervals. Incoming pages were recorded as well. RESULTS: Ten hospitalists were shadowed by a single research assistant for a total of 4467 minutes. Hospitalists spent 18% of their time on direct patient care, 69% on indirect patient care, 4% on personal activities, and 3% each on professional development, education, and travel. Communication accounted for 24% of the total minutes. Multitasking, performing more than one activity at the same time, was done 21% of the time. Hospitalists received an average of 3.4 +/- 1.5 pages per hour. CONCLUSIONS: Hospitalists spent most of their time on indirect patient care activities and relatively little time on direct patient care. Hospitalists spent a large amount of time on communication, underscoring the need for hospitalists to have outstanding communication skills and systems that support efficient communication. Multitasking and paging interruptions were common. The inherent distraction caused by interruptions and multitasking is a potential contributor to medical error and warrants further study.</description>

(28 February 1998), pp. 673-676.&lt;/i&gt;&lt;br /&gt;&lt;br /&gt;OBJECTIVE: An exploratory study to identify patterns of communication behaviour among hospital based healthcare workers. DESIGN: Non-participatory, qualitative observational study. SETTING: British district general hospital. SUBJECTS: Eight doctors and two nurses. RESULTS: Communication behaviours resulted in an interruptive workplace, which seemed to contribute to inefficiency in work practice. Medical staff generated twice as many interruptions via telephone and paging systems as they received. Hypothesised causes for this level of interruption include a bias by staff to interruptive communication methods, a tendency to seek information from colleagues in preference to printed materials, and poor provision of information in support of contacting individuals in specific roles. Staff were observed to infer the intention of messages based on insufficient information, and clinical teams demonstrated complex communication patterns, which could lead to inefficiency. CONCLUSION: The results suggest a number of improvements to processes or technologies. Staff may need instruction in appropriate use of communication facilities. Further, excessive emphasis on information technology may be misguided since much may be gained by supporting information exchange through communication technology. Voicemail and email with acknowledgment, mobile communication, improved support for role based contact, and message screening may be beneficial in the hospital environment.</description>

(c 2007), pp. 801-811.&lt;/i&gt;&lt;br /&gt;&lt;br /&gt;Several studies have shown that there is information loss during interruptions, and that multitasking creates higher memory load, both of which contribute to medical error. Nowhere is this more critical than in the emergency department (ED), where the emphasis of clinical decision is on the timely evaluation and stabilization of patients. This paper reports on the nature of multitasking and shift change and its implications for patient safety in an adult ED, using the methods of ethnographic observation and interviews. Data were analyzed using grounded theory to study cognition in the context of the work environment. Analysis revealed that interruptions within the ED were prevalent and diverse in nature. On average, there was an interruption every 9 and 14 min for the attending physicians and the residents, respectively. In addition, the workflow analysis showed gaps in information flow due to multitasking and shift changes. Transfer of information began at the point of hand-offs/shift changes and continued through various other activities, such as documentation, consultation, teaching activities and utilization of computer resources. The results show that the nature of the communication process in the ED is complex and cognitively taxing for the clinicians, which can compromise patient safety. The need to tailor existing generic electronic tools to support adaptive processes like multitasking and handoffs in a time-constrained environment is discussed.</description>

    <description>&lt;i&gt;Computer-Aided Design of User Interfaces IV (2005), pp. 271-282.&lt;/i&gt;&lt;br /&gt;&lt;br /&gt;The Ubiquitous Interactor (UBI) addresses the problems of design and development arising around services that need to be accessed from many different devices. In UBI, the same service can present different user interfaces on different devices by separating user-service interaction from presentation. The interaction is kept the same for all devices, and different presentation information is provided for different devices. This way, tailored user interfaces for many different devices can be created without multiplying development and maintenance work. In this paper we describe the system design of UBI, the system implementation, and two services implemented for the system: a calendar service and a stockbroker service.</description>

(September 2007), pp. 759-769.&lt;/i&gt;&lt;br /&gt;&lt;br /&gt;When mobile devices are used on the move, a user's limited visual resources are split between interacting with the mobile devices and maintaining awareness of the surrounding environment. In this study, we examined stylus-based tapping operations on a PDA under three mobility situations: seated, walking on a treadmill, and walking through an obstacle course. The results revealed that Fitts' Law continues to be effective even under the most challenging obstacle course condition. While target selection times did not differ between the various mobility conditions, overall task completion times, error rates, and several measures of workload differed significantly. Diminished performance under the obstacle course condition was attributed to increased demands on attention associated with navigating through the obstacle course. Results showed that the participants in the obstacle course condition were able to tap on a 6.4 mm-diameter target with 90% accuracy, but they reduced their walking speed by 36% and perceived an increased workload. Extending earlier research, we found that treadmill-based conditions were able to generate representative data for task selection times, but accuracy differed significantly from the more realistic obstacle course condition.</description>
