AbstractHospitals must recognize that ED crowding is a hospital-wide problem, not an ED problem. Multi-disciplinary, hospital-wide teams are essential to overseeing and implementing change. “champion” for change must be identified or cultivated in the institution Senior leadership needs to send a clear and consistent message that improving patient flow is a priority. Hospitals must learn and use formal improvement methods, like rapid cycle change, on a daily basis and track results. Institutions must commit themselves to using rigorous metrics, because “we can’t fix what we can’t measure.” Most hospitals collect a limited amount of useful patient flow data. Transparency around initiatives and data must become an organizational value, so that all stakeholders have the information they need to do their jobs.
